Propuesta para la medición de cargas de trabajo de enfermería en UCI hospital universitario Donostia

Abstract

Introducción: La valoración de las cargas de trabajo de enfermería en las Unidades de Cuidados Intensivos (UCI) puede llegar a ser una herramienta útil para la mejor dotación/repartición de personal por turnos, lograr un menor coste económico junto a una mayor calidad y seguridad clínica y mejor así atención al paciente y su entorno. Logrando, una mayor satisfacción por parte del personal de enfermería. Objetivos: Visualizar la trayectoria de las herramientas de cálculo de carga de trabajo en cuidados intensivos, determinando cuál es la mejor herramienta en la actualidad. Intentando adaptar dicha herramienta a la unidad de Cuidados Intensivos del Hospital Universitario Donostia (HUD). Observando los puntos de mejora que podría aportarse al servicio. En este estudio se valoraran la Therapeutic Intervention Scoring System (TISS-28), la Nine Equivalent of Nursing Manpower Score (MENS) y el Nursing Activities Score (NAS) para el cálculo de cargas de enfermería. Métodos: Se hará una revisión bibliográfica teniendo en cuenta los mejores artículos originales y meta análisis más citados de los últimos 20 años sobre las cargas de trabajo relacionadas con los servicios de cuidados intensivos. Conclusión: Los resultados de este estudio exponen que la herramienta capaz de medir la carga de trabajo de enfermería de forma directa, y de manera más exacta (81%), es el NAS. Siendo ésta la única capaz de medir dicha carga por tunos de trabajo (modelo Español). Así mismo, se tendría que modificar esta herramienta para su utilización en la UCI del HUD. De este estudio, se desprenden áreas de mejora relacionada con este sistema de cálculo de cargas de trabajo para su implantación en UCI de HUD

Introduction: The scaled evaluation of different nursing roles in the Intensive Care Unit (ICU) can be useful in the better distribution of personnel for shifts, in achieving a lower economic cost and in better quality, increasing clinical security and bettering the patient care and environment. This leads to achieving a better satisfaction thanks to the nursing personnel. Objectives: To visualize the trajectory of the different tools used to calculate the nursing loads for work in intensive care, determining which tools are best for the present situation. In this manner, we can use the tool to identify points of improvement. In this study we are going to use the following scoring systems to define the nursing work loads at the ICU, Hospital Universitario Donostia: the Therapeutic Intervention Scoring System (TISS), the Nine Equivalent of Nursing Manpower Score (MENS) and the Nursing Activities Score (NAS). Methods: A biographical review will be done taking into account the best original articles and meta analysis, from the last 20 years, concerning the nursing work loads related to the Intensive Care Unit. Conclusion: The results in this study shows that the best tool for measuring the nursing work loads in the most direct and accurate manner (81%), is the NAS. Being that this is the only scale capable of measuring load by work shifts. The areas of improvement achieved by this study are related to: The reorganization of the nursing personnel in the different shifts, grouping of patients according to pathology, the creation of an intermediate care unit, and a better management of the ICU patients
