Publication Open Access Meta-analysis of prevalence of wheezing and recurrent wheezing in infants(Elsevier, 2016) Álvarez Álvarez, Ismael; Niu, Hao; Guillén Grima, Francisco; Aguinaga Ontoso, Inés; Ciencias de la Salud; Osasun ZientziakBackground: Wheezing affects children’s quality of life, and is related with asthma in childhood. Although prevalence of wheezing has been previously studied in several countries, there are no reference of worldwide prevalence in infants. The aim of this meta-analysis is to estimate the prevalence of wheezing and recurrent wheezing in infants aged up to two years, and compare the prevalence across world regions. Methods: Literature search was conducted in MEDLINE and SCOPUS databases, looking for observational studies published up to June 2016, including as keywords “prevalence” or “epidemiology” combined with “wheeze”, “wheezing” or “asthma symptoms” and “infant” or “preschool”. Fast*Pro software and random effects Bayesian model were used. Heterogeneity was estimated using I2 statistic, and sensitivity analyses were performed. Results: We identified 109 studies after duplicates were removed. After exclusions, 14 studies were included in the meta-analysis. Prevalence of wheezing and recurrent wheezing were 36.06% (95% CI 35.17-36.96), and 17.41% (95% CI 16.74-18.09), respectively. In European countries, prevalence of wheezing was 30.68% (95% CI 28.97-32.45), and 12.35% (95% CI 11.27-13.47) for recurrent wheezing. Prevalence of wheezing and recurrent wheezing in Latin America were higher, 40.55% (95% CI 39.40-41.71), and 19.27% (95% CI 18.44-20.11), respectively. In Africa, prevalence of wheezing was 15.97% (95% CI 14.05-18.00). Low or no heterogeneity was found in all cases. Conclusions: More than one third of infants suffer from wheezing and almost one fifth from recurrent wheezing, being these illnesses especially prevalent in Latin American countries, pointing out an important public health problem.Publication Open Access Negative consequences of substance use in European university students: results from Project SNIPE(Karger, 2020) McAlaney, John; Dempsey, Robert C.; Helmer, Stefanie M.; Hal, Guido van; Bewick, Bridgette M.; Akvardar, Yildiz; Guillén Grima, Francisco; Ciencias de la Salud; Osasun ZientziakBackground: University students are a risk group for heavy substance use and the experience of various potentially severe negative substance use consequences which may impact on their health, social, and academic functioning. Whilst the experience of negative consequences of substance use is well understood in North American student samples, there is little data on these experiences in European students. In order to develop effective harm prevention and reduction interventions for students' substance use, there needs to be an understanding of the types of consequences experienced in European student samples. Objectives: The aim of the study was to investigate the prevalence and predictors of the experience of negative substance use-related consequences amongst university students in 7 European countries. Methods: University students (n = 4,482) in Belgium, Denmark, Germany, the Slovak Republic, Spain, Turkey, and the UK completed an online survey of their substance use behaviours and the experience of associated negative consequences. Results: European students reported that experiencing a hangover or illness, missing class, being short of money, and experiencing memory loss were the most commonly experienced negative consequences of substance use. Not living with other students and using alcohol, cannabis, sedatives, and cocaine were also associated with higher odds of experiencing these negative consequences. Conclusions: In contrast to North American data, European university students tended to experience consequences that are associated with lower level health risks rather than more severe consequences (e.g., drink-driving and physical injury). Harm prevention and reduction interventions for students should be targeted towards those consequences that are most salient to the target group to ensure feedback is relevant and potentially more effective in changing students' substance use behaviours.Publication Open Access Normative misperceptions of tobacco use among university students in seven European countries: baseline findings of the 'Social Norms Intervention for the prevention of Polydrug usE' study(Elsevier, 2015) Pischke, Claudia R.; Helmer, Stefanie M.; McAlaney, John; Bewick, Bridgette M.; Guillén Grima, Francisco; Ciencias de la Salud; Osasun ZientziakIntroduction: Research conducted in North America suggests that students tend to overestimate tobacco use among their peers. This perceived norm may impact personal tobacco use. It remains unclear how these perceptions influence tobacco use among European students. The two aims were to investigate possible self-other discrepancies regarding personal use and attitudes towards use and to evaluate if perceptions of peer use and peer approval of use are associated with personal use and approval of tobacco use. Methods: The EU-funded 'Social Norms Intervention for the prevention of Polydrug usE' study was conducted in Belgium, Denmark, Germany, Slovak Republic, Spain, Turkey and United Kingdom. In total, 4482 students (71% female) answered an online survey including questions on personal and perceived tobacco use and personal and perceived attitudes towards tobacco use. Results: Across all countries, the majority of students perceived tobacco use of their peers to be higher than their own use. The perception that the majority (> 50%) of peers used tobacco regularly in the past two months was significantly associated with higher odds for personal regular use (OR: 2.66, 95% CI: 1.90–3.73). The perception that the majority of peers approve of tobacco use was significantly associated with higher odds for personal approval of tobacco use (OR: 6.49, 95% CI: 4.54–9.28). Conclusions: Perceived norms are an important predictor of personal tobacco use and attitudes towards use. Interventions addressing perceived norms may be a viable method to change attitudes and tobacco use among European students, and may be a component of future tobacco control policy.Publication Open Access Relationship between perceived body weight and body mass index based on self- reported height and weight among university students: a cross-sectional study in seven European countries(BioMed Central, 2010) Mikolajczyk, Rafael T.; Maxwell, Annette E.; El Ansari, Walid; Stock, Christiane; Petkeviciene, Janina; Guillén Grima, Francisco; Ciencias de la Salud; Osasun ZientziakBackground: Despite low rates of obesity, many university students perceive themselves as overweight, especially women. This is of concern, because inappropriate weight perceptions can lead to unhealthy behaviours including eating disorders. Methods: We used the database from the Cross National Student Health Survey (CNSHS), consisting of 5, 900 records of university students from Bulgaria, Denmark, Germany, Lithuania, Poland, Spain and Turkey to analyse differences in perceived weight status based on the question: "Do you consider yourself much too thin, a little too thin, just right, a little too fat or much too fat?". The association between perceived weight and body mass index (BMI) calculated from self-reported weight and height was assessed with generalized non-parametric regression in R library gam. Results: Although the majority of students reported a normal BMI (72-84% of males, 65-83% of females), only 32% to 68% of students considered their weight "just right". Around 20% of females with BMI of 20 kg/m(2) considered themselves "a little too fat" or "too fat", and the percentages increased to 60% for a BMI of 22.5 kg/m(2). Male students rarely felt "a little too fat" or "too fat" below BMI of 22.5 kg/m(2), but most felt too thin with a BMI of 20 kg/m(2). Conclusions: Weight ideals are rather uniform across the European countries, with female students being more likely to perceive themselves as "too fat" at a normal BMI, while male students being more likely to perceive themselves as "too thin". Programs to prevent unhealthy behaviours to achieve ill-advised weight ideals may benefit students.Publication Open Access Cost efectiveness analysis of therapeutic strategies for patients with chronic hepatitis C previously non responders to interferon(2003) San Miguel Elcano, Ramón; Mar, Javier; Cabasés Hita, Juan Manuel; Guillén Grima, Francisco; Butí, María; Economía; EkonomiaBackground: The efficacy of combination therapy in previous non responders to interferon (IFN) monotherapy with chronic hepatitis C is lower than in naïve patients, and there has been no economic evaluation in this population. Aim: To develop a cost-effectiveness analysis of therapeutic regimens with IFN alpha and ribavirin in previous interferon non-responders. Methods: A Markov simulation model was used to project the clinical and economic outcomes of five different therapeutic strategies including a “no treatment” alternative using the health care system perspective. The effectiveness data for the different doses and durations was obtained from a previously performed meta-analysis. A sensitivity analysis was performed to test robustness of the model, analysing changes in different variables. Results: Applying a 3% discount rate, the standard patient on combination therapy for 12 months showed an increase of 0.80 years and 1.55 quality adjusted life years (QALYs), when comparing combination therapy for 12 months vs. “no treatment” strategy. This option led to an incremental cost-effectiveness ratio of 11,767 euros per year of life gained and 6,073 euros per QALY. Conclusions: Combination therapy with interferon plus ribavirin in previous interferon non-responders shows an incremental cost-effectiveness ratio within the range of some well accepted medical interventions in our health care system.Publication Open Access Prevalencia e incidencia de la enfermedad de Alzheimer en Europa: metaanálisis(Elsevier España, 2017) Niu, Hao; Álvarez Álvarez, Ismael; Guillén Grima, Francisco; Aguinaga Ontoso, Inés; Ciencias de la Salud; Osasun ZientziakIntroducción: La enfermedad de Alzheimer es el principal tipo de demencia, y una enfermedad de etiología desconocida. Con el aumento de la población anciana mundial, el número de pacientes con enfermedad de Alzheimer muestra una tendencia de crecimiento rápido. El objetivo de este metaanálisis es evaluar la prevalencia e incidencia de la enfermedad de Alzheimer en Europa. Metodología: La búsqueda de artículos se realizó en las bases de datos Medline, Scopus y CINAHL Complete utilizando las palabras claves “Alzheimer”, “Alzheimer’s disease” y “AD”; combinadas con “prevalence”, “incidence” y “epidemiology”. Se utilizó el modelo Bayesiano de efectos aleatorios, mostrando intervalos de credibilidad al 95%. Para estimar la heterogeneidad se usó el estadístico I2. Resultados: La prevalencia de enfermedad de Alzheimer en Europa fue 5,05% (IC 95% 4,73-5,39). La prevalencia por sexo en los hombres y las mujeres fue 3,31% (IC 95% 2,85-3,80) y 7,13% (IC 95% 6,56-7,72), respectivamente, y se encontró una tendencia creciente por grupos de edad. La incidencia de enfermedad de Alzheimer en Europa fue 11,08 por 1000 personas-año (IC 95% 10,30-11,89), siendo en los hombres y las mujeres de 7,02 por 1000 personas-año (IC 95% 6,06-8,05) y 13,25 por 1000 personas-año (IC 95% 12,05-14,51), respectivamente, con igual tendencia creciente con el aumento de la edad. Aim: To examine the development between 2005 and 2012 of three specialised palliative care services across the World Health Organization European Region – home care teams, hospital support teams and inpatient palliative care services. Design and setting: Data were extracted and analysed from two editions of the European Association for Palliative Care Atlas of Palliative Care in Europe. Significant development of each type of services was demonstrated by adjusted residual analysis, ratio of services per population and 2012 coverage (relationship between provision of available services and demand services estimated to meet the palliative care needs of a population). For the measurement of palliative care coverage, we used European Association for Palliative Care White Paper recommendations: one home care team per 100,000 inhabitants, one hospital support team per 200,000 inhabitants and one inpatient palliative care service per 200,000 inhabitants. To estimate evolution at the supranational level, mean comparison between years and European sub-regions is presented. Results: Of 53 countries, 46 (87%) provided data. Europe has developed significant home care team, inpatient palliative care service and hospital support team in 2005–2012. The improvement was statistically significant for Western European countries, but not for Central and Eastern countries. Significant development in at least a type of services was in 21 of 46 (46%) countries. The estimations of 2012 coverage for inpatient palliative care service, home care team and hospital support team are 62%, 52% and 31% for Western European and 20%, 14% and 3% for Central and Eastern, respectively. Método: Ciento seis sujetos de un estudio de intervención dietética fueron evaluados personalmente con el MMSE y por teléfono con la STICS-m. La correlación entre ambos se midió con el coeficiente de correlación intraclase (CCI) de consistencia. Además, 932 participantes mayores de 55 años de la cohorte 'Seguimiento Universidad de Navarra' fueron evaluados con la STICS-m. Durante una mediana de seguimiento de 6,5 años, se recogió información sobre el desarrollo de demencia. Mediante regresión logística se estudió la asociación entre la puntuación de la STICS-m o el cambio a 2 años en la puntuación y el riesgo de desarrollar demencia, ajustando por apolipoproteína E, edad y años de educación universitaria. Resultados: El CCI entre el MMSE y la STICS-m fue de 0,31 (intervalo de confianza del 95% [IC95%]:0,13-0,48). La odds ratio (OR) ajustada para el desarrollo de demencia para cada punto adicional en la puntuación basal de la STICS-m fue de 0,85 (IC95%: 0,72-1,02; p = 0,084). Al considerar el cambio en la puntuación a los 2 años como variable independiente, la OR fue de 0,79 (IC95%: 0,67-0,93; p = 0,006).Publication Open Access Perfil de la demanda urgente e influencia del fútbol televisado en un servicio extrahospitalario en la Zona Básica de Salud de Tafalla, Navarra(Ministerio de Sanidad y Consumo, 2003) Pérez Ciordia, Ignacio; Catalán Fabo, Francisca; Zalacain Nicolay, Fernando; Barriendo Antoñanzas, Maite; Solaegui Díaz de Guereñu, Ramón; Guillén Grima, Francisco; Ciencias de la Salud; Osasun ZientziakFundamento: la demanda a los servicios de urgencias, tanto hospitalarios como extrahospitalarios, mantiene un ritmo creciente. El objetivo del presente trabajo es doble: cuantificación y estudio de las características personales de los usuarios demandantes de atención urgente y valorar si el fútbol televisado influye en la utilización del servicio de urgencias. Métodos: Estudio descriptivo longitudinal (9.723 usuarios demandantes) y estudio de casos y controles (1.284 usuarios demandantes) según presencia o no de fútbol televisado mediante modelo de regresión logística. Las asociaciones se han cuantificado mediante la odds ratio (OR). Se excluyen del estudio las consultas telefónicas y las de enfermería. Resultados: El 10,6% de la demanda ha correspondido a consultas en domicilio y se han remitido al hospital el 4,8% del total. El 13,3% de la demanda corresponde a personas desplazadas de otras zonas de salud y son 65 los usuarios hiperfrecuentadores con 8 o más consultas. El mes de agosto (32,3%), el domingo (44,56%) y el grupo horario de 12 a 14 horas (8,38%) son los momento de mayor demanda de atención, siendo las diferencias estadísticamente significativas. El fútbol televisado se asoció a un aumento de la demanda del 19,8% (p<0,001) respecto al periodo control. Conclusiones: Se observa una alta frecuentación a los servicios de urgencia extrahospitalaria, con una importante concentración de la demanda en momentos muy concretos. Es muy importante la atención prestada a personas desplazadas. La retransmisión de fútbol televisado se asocia con una mayor utilización del servicio de urgencias.
