    Suicidal ideation and attempts among patients with lifetime physical and/or sexual abuse in treatment for substance use disorders

    Background: Several studies indicate that lifetime abuse is a relevant risk factor for suicidal ideation and/or attempts. However, little is known about this phenomenon in patients seeking treatment for substance use disorder. The prevalence rate of suicidal ideation and/or suicide attempts was explored among lifetime physically and/or sexually abused patients receiving treatment for drug addiction. The differential characteristics between these patients and those without suicidal behaviours were studied. Method: Three hundred and seventy-five patients were assessed. Socio-demographic characteristics, addiction severity, lifetime abuse, suicidal ideation and attempts, and psychopathological symptoms were explored. Results: Eighty-two patients (21.9%) presented with a history of lifetime abuse and were included in the study (37 men and 45 women). Sixty-two per cent of them presented with lifetime suicidal ideation (12.2% in the last month), and 30.5% with suicide attempts (1.2% in the last month). Patients with suicidal ideation or attempts showed a more severe addiction profile (assessed by the EuropASI) and more psychopathological symptoms (assessed by the SCL-90-R). Conclusion: This study highlights the relationship between previous traumatic experiences and suicidal behaviours. According to these results, systematic screening of suicidal risk in patients seeking treatment in addiction centres with histories of abuse is recommended.

    Profiles of mothers who seek treatment for substance use disorders in a clinical centre

    Purpose: The purpose of this paper is to explore the prevalence and profile of mothers among women who sought treatment for drug addiction, as well as the therapeutic progression of these patients. Design/methodology/approach: A sample of 180 Spanish women with addiction problems was assessed. Information was collected on the patients’ socio-demographic characteristics, consumption variables and psychological symptoms. Findings: Of the total sample, 22.2 per cent (n = 40) of the women seeking treatment for substance use disorder were mothers. Compared with women without children, mothers scored significantly higher on several EuropASI and psychological variables. Specifically, mothers presented with more medical problems, worse employment/financial situations, and more severity in alcohol use. Moreover, having a history of lifetime physical and/or sexual abuse was related to belonging to the group of mothers. Regarding therapeutic progression, no statistically significant differences in the retention rate were found between mothers and non-mothers. Originality/value: The results of this study show that mothers generally have more severe problems than non-mothers. Therefore, comprehensive, continuum-based, and client-centred care for mothers is paramount for effective treatment in mothers. The implications of these results for further research and clinical practice are discussed.

    Determining socio-demographic predictors of treatment dropout: results in a therapeutic community

    In this paper, a study about sociodemographic factors which predict treatment dropout from a residential substance use disorder program in Spain (Proyecto Hombre Therapeutic Community), is presented. A sample of 430 patients (173 treatment completers and 257 treatment dropouts) was assessed at entry to therapeutic community program in order to collect information about sociodemographic characteristics of the participants. According to the obtained results, factors that predicted treatment dropout included younger age, male gender, and to have previous treatment history in the Proyecto Hombre therapeutic community. Patients with these characteristics were at significantly greater risk of dropping out of treatment. Implications of these results for further research and clinical practice are commented upon.

    El compuesto psicoactivo de la cannabis sátiva, el í-9-tetrahidrocannabinol, ejerce sus efectos sobre el sistema nervioso central a través del receptor cannabinoide CB1. La localización presináptica del receptor CB I sugiere una función de modulación de la liberación de neurotransmisores a través de la denominada señalización retrógrada. El THC actúa en el sistema de recompensa cerebral de una manera muy similar a la de otras sustancias adictivas, incluyendo tanto la capacidad de generar tolerancia y síndro­ me de abstinencia como la interacción con otros sistema de neurotransmisión implica­ dos en el fenómeno de la recompensa. El consumo de cannabis provoca, al menos de manera transitoria, déficit cognitivo en funciones como atención, memoria, habilidades verbales y resolución de problemas. También puede provocar síntomas psicóticos y aumentar el riesgo de padecer esquizofrenia en sujetos predispuestos. El consumo de cannabis aumenta la probabilidad de sufrir síntomas depresivos y de ansiedad, princi­ palmente en los consumidores de altas dosis. Sin embargo, no se ha logrado evidencia suficiente que confirme el papel causal del cannabis sobre el síndrome amotivacional, aunque sí se ha relacionado el consumo de THC con un menor nivel educacional y mayor uso de otras drogas ilegales. Desde una perspectiva evolucionista, las drogas actúan en regiones cerebrales filogenéticamente antiguas, lo cual señala la vulnerabili­ dad del ser humano ante las conductas adictivas. La predisposición genética modifica la expresión de dicha vulnerabilidad y se ve influida por las experiencias, que modifican o facilitan la transcripción del gen.

    Profile of addicted patients who re-enter treatment programmes

    Objective. This study explored the differential profile of addicted patients who re-enter treatment programmes. Method. A sample of 252 addicted patients (203 male and 49 female) who sought outpatient treatment was assessed. Data regarding socio-demographic factors, drug consumption factors (assessed using the EuropAsi), psychopathological factors (assessed using the Symptom Checklist 90 Revised [SCL-90-R]), and personality variables (assessed using the Millon Clinical Multiaxial Inventory II [MCMI-II]) were collected. Results. 65.9% (n=166) of drug-addicted patients were re-admitted into treatment programmes. All of the variables for which we collected data were compared between these treatment repeaters and patients who were admitted for the first time. Significant differences between the two groups of patients were found for some of the variables that we examined. Treatment repeaters were generally older and had a poorer employment situation than first-time admits. Treatment repeaters were also more likely to report poly-consumption and to have sought treatment for alcohol abuse. Moreover, some of the scores for several EuropAsi, SCL-90-R, and MCMI-II variables were statistically significantly different from those of the first-time admits. Conclusions. According to these results, patients who re-enter treatment programmes often present with more severe addiction problems. The implications of these results for further research and clinical practice are discussed.

    Gender differences in drug-addicted patients in a clinical treatment center of Spain

    Background and objectives: This study explored the characteristics of a representative sample of patients who were addicted to drugs and analyzed the differential profile of addicted women and men. Methods: A sample of 195 addicted patients (95 female and 100 male) who sought outpatient treatment in a Spanish clinical center was assessed. Information on sociodemographic, consumption and associated characteristics was collected using the European Addiction Severity Index (EuropASI). Results: The results showed statistically significant differences between groups. Demographically, the differences were centered on employment, with more labor problems in the female group. Regarding addiction severity, the EuropASI results showed statistically significant differences in both the Interviewer Severity Ratings (ISR) and Composite Scores (CS). Women experienced more severe impacts in the medical, family social and psychiatric areas. By contrast addicted men had more severe legal problems than addicted females did. Conclusions: According to these results, women who seek outpatient treatment in a clinical center presented with a more severe addiction problem than men did. Moreover, they reported more significant maladjustment in the various aspects of life explored.

    Gender differences in treatment progress of drug-addicted patients

    This study explored the differences in treatment progress between men and women who were addicted to drugs. The differential rate of completion of/dropout from treatment in men and women with substance dependence was established. Moreover, comparisons between completers and dropouts, accounting for gender, were carried out for several variables related to treatment progress and clinical profile. A sample of 183 addicted patients (96 male and 87 female) who sought outpatient treatment between 2002 and 2006 was assessed. Information on socio-demographic, consumption and associated characteristics was collected. A detailed tracking of each patient's progress was maintained for a minimum period of eight years to assess treatment progression. The treatment dropout rate in the whole sample was 38.8%, with statistically significant differences between women (47.1%) and men (31.3%). Women who dropped out of treatment presented a more severe profile in most of the psychopathologic variables than women who completed it. Moreover, women who dropped out from treatment presented a more severe profile than men who dropped out. According to these results, drug-addicted women showed worse therapeutic progress than men with similar histories. Thus, women must be provided with additional targeted intervention to promote better treatment outcomes.

    Deshabituación de drogas y funcionamiento cerebral: una visión integradora

    En la actualidad todavía estamos lejos de contar con modelos de comprensión y tratamiento de las adicciones que se sustenten en el funcionamiento cerebral. El objetivo de este trabajo es elaborar un modelo integrador entre psicología clínica y neuropsicología con el fin de anclar nuestras intervenciones clínicas en enfoques que nos acerquen al discurso de la neurociencia. Más en concreto, se revisan tres modelos de intervención de amplia utilización en adicciones como son la entrevista motivacional, la prevención de recaídas y el trabajo con redes sociales. Posteriormente, se relacionan estos enfoques con su posible efecto en el funcionamiento cerebral, basándonos en el reconocido “modelo jerárquico” de Stuss y Benson y en el modelo de circuitos cerebrales propuesto por Volkow, Fowler y Wang. Así cada intervención tendría como objetivo eliminar diversas funciones cerebrales que se ven modificadas durante el proceso adictivo. Las intervenciones propias de la entrevista motivacional mejorarían el funcionamiento del nivel superior de procesamiento cerebral, aumentando la conciencia del problema. Estrategias del trabajo con redes sociales como la detección de situaciones de riesgo o la evitación de estímulos condicionados incidirían en los niveles de procesamiento básico y de control de activación de acciones. Técnicas propias de la prevención de recaídas como la exposición con prevención de respuesta o el manejo del E.V.A. influirían en el funcionamiento de los niveles de autocontrol y de funciones básicas. Además, estos enfoques de tratamiento también modificarían los circuitos de recompensa, motivación, memoria y control cognitivo. En definitiva, la finalidad es someter al sujeto una serie de experiencias para que las relaciones cerebro-mente se orienten en una dirección más adaptativa.

    Prevalence of pathological gambling in treatment-seeking addicted patients: an exploratory study with the South Oaks Gambling Screen

    En este estudio se lleva a cabo un análisis de la prevalencia del juego patológico en 112 pacientes adictos (81 alcohólicos y 31 dependientes de la cocaína) que acuden en busca de tratamiento. Para ello, se utilizaron los criterios diagnósticos del DSM-IV-TR para el juego patológico y la versión española del Cuestionario de Juego Patológico de South Oaks (SOGS). Los resultados obtenidos mostraron que el 22,3% de los pacientes drogodependientes estudiados presentaba un diagnóstico comórbido de ludopatía. Además, un 11,6% adicional obtenía una puntuación en el SOGS indicadora de juego problemático. En suma, el 33,9% de la muestra presentaba síntomas de juego clínicamente significativos. La comparación entre los pacientes adictos con y sin ludopatía asociada mostró diferencias significativas en las variables relacionadas con el consumo de alcohol (evaluado mediante el EuropASI), los síntomas psicopatológicos (evaluados mediante el SCL-90-R) y algunas variables de personalidad (evaluadas mediante el MCMI-II). En todos los casos, las puntuaciones eran significativamente más altas en los pacientes ludópatas que en los que no tenían un problema de ludopatía asociado. Se comentan las implicaciones de este estudio para la práctica clínica y la investigación futura.

    Evaluation of a therapeutic community treatment of addictions: a long-term follow-up study in Spain

    The aims of this paper were to carry out a long-term follow-up evaluation of a well-established therapeutic community treatment for addictions in Navarre (Spain) and to make a comparison between the program completers and dropouts, as well as between relapsing and non-relapsing patients, on a wide set of variables. A long-term follow-up design (mean of 6 years after leaving treatment) was used to analyze the outcomes of the therapeutic program. The sample consisted of 155 subjects (113 completers and 42 dropouts). A personal interview was carried out with each one of the located subjects. The interviews took place between September 2000 and September 2004. Treatment "dropouts" manifested a higher and earlier rate both of relapses, and of new treatments for their drug addiction than the completion group. The program was also effective in reducing criminal behavior and improving the state of health. Significant differences were found across outcome variables when comparison was made between treatment completers and "dropouts". All subjects improved on outcome variables after receiving the treatment. When patients with and without relapse were compared, significant outcome differences were also found between groups. The study's limitations are noted and future needed research is suggested.
