    El programa Health-Friendly tiene como objetivo reducir el miedo al entorno sanitario de los niños y niñas entre 5 y 9 años. Es un programa de inmersión basado en una intervención en la que los niños y niñas, a través de un entorno sanitario simulado, toman contacto con distintos escenarios, como una consulta de enfermería o una habitación de hospitalización quirúrgica. En el proyecto de diseño y evaluación del programa Health-Friendly participaron niños y niñas de distintos colegios de Navarra. La intervención se llevó a cabo en el Espacio de Habilidades Clínicas y Simulación de la Facultad de Ciencias de la Salud de la Universidad Pública de Navarra (UPNA).

    Assessing the reliability and validity of the spanish version of the actual scope of nursing practice scale

    Nursing workforce shortage is one of the main challenges for healthcare organizations and it is important to determine if nurses are fulfilling their full scope of practice. There is a questionnaire that measures the activities carried out by nurses, but there is no version available for the Spanish context. The purpose of this study was to develop a cross-cultural adaptation of the “Actual Scope of Nursing Practice” questionnaire by D’Amour et al., and to assess the psychometric characteristics of the Spanish version. An exploratory sequential research design was used. The cross-cultural adaptation was performed using translation, back-translation, review, and pre-testing. Psychometric properties were assessed to determine its construct validity and internal consistency. Among the 501 eligible nurses from the three main hospitals in the region, the first 310 nurses to respond to an online questionnaire were included in our study. The response rate was 61.9%. They were invited via email and completed the survey using the SurveyMonkey platform. The Spanish version of the questionnaire was obtained. A final scale with twenty items and two factors was confirmed with an adequate fit, with the item scores demonstrating that all items were optimally related to their respective latent construct. The alpha coefficients for the Spanish ASCOP scale were robust and revealed good internal consistency. This study showed that the Spanish version of the scale, “Scope of Nursing Practice”, has a good degree of validity and reliability. This questionnaire can support nurse managers in realising nursing activities within their organisations and in promoting desirable work outcomes among nurses.

    Impact of individual and job characteristics on nurses' scope of practice in spanish hospital units

    Background. Nurses are one of the largest and costliest groups in healthcare organizations; therefore, it is important to comprehend their scope of practice. Aim. To contribute to the improvement of nursing resource management in medical-surgical and Intensive Care Units by identifying factors that influence the scope of nursing practice. The hypothesis was that the activities carried out by nurses in medical-surgical units and intensive care units are influenced by individual and job-related factors, with job characteristics having an additive and moderating effect on individual characteristics. Materials and Methods. Cross-sectional correlational design to test the relationship between the individual and job characteristics on the nursing scope of practice measured by the Actual Scope of Nursing Practice (ASCOP) questionnaire. The sample consisted of 270 nurses. Linear mixed effects models analysis (LME) was used with the aleatory effect of the intensive care unit (ICU). Results. Belonging to the ICU decreased the scope of practice of nurses. We found a statistically significant effect of psychological demand, practice environment, role ambiguity, and growth need strength on the scope of nursing practice. The models explained a variance up to 24%. Conclusions. Although the survey results revealed the existence of broader scope of practice levels in Spanish hospital units than in the original Canadian study, the use of scope of nursing practice remains suboptimal. Higher levels on the psychological demand, the practice environment and in the individual growth need strength were related with a broader scope of practice. Otherwise, role ambiguity negatively affected the scope of practice. Implications for Nursing Management. This article provides an analysis of the impact of individual and job-related characteristics on the nursing scope of practice. It serves as a valuable resource for both managers and nurses, offering insights to improve nurses' working conditions and obtain more efficient workforces.

    The Spanish version of the child medical fear questionnaire: cross-cultural adaptation and validation

    Having valid and reliable tools that help health professionals to assess fear in children undergoing medical procedures is essential to offer humanised and quality of care in the paediatric population. The aim of this study was to develop the cross-cultural adaptation and the evaluation of the psychometric properties of the Spanish version of the 'Child Medical Fear Scale' in its shortened version (CMFS-R). The design consisted of two phases: first, of cross-cultural adaptation and second, of the psychometric validation of the CMFS-R with a sample of 262 children from Spain, applying a cross-sectional design. Confirmatory factor analysis was conducted to assess construct validity and the Cronbach's alpha and the adjusted item-total score correlation coefficients were performed to study reliability. The results confirmed internal consistency and construct validity of the Spanish version of the CMFS-R, indicating that the scale has an acceptable level of validity and reliability. Therefore, this study brings a new version of the scale to assess fear related to medical procedures for use in the Spanish paediatric population.

    Gestión del cambio en una Unidad de Cuidados Intensivos

    El régimen de visitas en una Unidad de Cuidados Intensivos (UCI) es un tema que lleva años en discusión ya que a pesar de que las recomendaciones apunten hacia un régimen de visitas abierto, la realidad es que en la mayoría de las UCI siguen existiendo políticas restrictivas de visitas y presencia de familiares. Ante un inmediato cambio de la UCI a una nueva estructura física que proporciona las condiciones estructurales ideales para proponer una flexibilización del régimen de visitas, se fijó como objetivo general proponer una estrategia para incorporar una participación más activa de la familia en el cuidado del paciente crítico. Los objetivos específicos fueron los siguientes: 1. Elaborar un documento con información dirigida a facilitar la adaptación al medio de la hospitalización en Cuidados Intensivos a los familiares del paciente crítico. 2. Conocer las posturas y creencias de los profesionales respecto a los diferentes modelos para las visitas. 3. Trabajar con los profesionales aportándoles conocimientos que les ayuden a superar las barreras relacionadas con el establecimiento de un régimen de visitas más flexible. Para conocer el estado de la cuestión previo a plantear ninguna intervención, se realizó un análisis estratégico que contó con una revisión de la evidencia existente sobre el tema, una observación de la realidad del régimen de visitas en el hospital y la descripción de la Unidad actual, todo ello como parte del análisis externo. Con la finalidad de analizar la Unidad desde el interior, se realizó un cuestionario validado para conocer las creencias que el equipo de enfermería de la Unidad de Cuidados Intensivos tiene sobre el régimen de visitas abierto. Con las conclusiones obtenidas, se han desarrollado dos propuestas de intervención. Una documento informativo para familiares de pacientes, que facilitará la adaptación de los mismos al medio de UCI y por otro lado el diseño de una actividad formativa dirigida a los profesionales, para llevarla a cabo con el objetivo de facilitar la instauración de un régimen de visitas más permisivo.

    HIV Incidence Among Women in Sub-Saharan Africa: a time trend Aanalysis of the 2000-2017 Period

    ABSTRACT: The aim of this study was to use data from the United Nations Global Indicators Database to analyze the trends in the HIV incidence rate among women in sub-Saharan African countries between 2000 and 2017. The HIV incidence rate is defined as the number of new HIV infections per 1,000 uninfected population, aged 15 to 49 years old. Joinpoint regression analysis was applied to identify periods when there were significant changes in the HIV incidence rate. The results show that there was a global decrease trend in the HIV incidence rates among women in sub-Saharan Africa, decreasing in all sub-Saharan African countries, except in Angola, Equatorial Guinea, and Sudan, which have remained the same, and Madagascar, where the overall trend is increasing. The joinpoint regression statistical method offers an in-depth analysis of the incidence of HIV among women in sub-Saharan Africa. Copyright © 2021 Association of Nurses in AIDS Care.

    Density of nurses and midwives in sub-Saharan Africa: trends analysis over the period 2004-2016

    Aim: to analyse the trends of density rates of nurses and midwives per population in sub-Saharan Africa over the period from 2004 to 2016. Background: nursing, the largest health care workforce, is actively contributing to the achievement of the Sustainable Development Goals. The African continent is one of the most affected areas by the differences in the density of nurses and midwives indicator. Methods: joinpoint regression analysis was applied to identify significant changes in trends of the density of nurses and midwives from the 50 countries of sub-Saharan Africa. Results: from 2004 to 2013, the density of nurses and midwives in sub-Saharan Africa increased significantly from 5.6 to 12.44 per 10,000 population, although it exhibited a increasing trend of a magnitude of 8.3% until 2013 that does not continue from that year. Conclusions: only seven countries show an increasing trend, although in the case of the rest, they do not present any trend that suggests a change in this indicator in the short term. Implications for Nursing Management: from a macrolevel nursing management point of view, our study shows the importance of implementing actions that contribute to the increase of the nursing workforce in Africa, essential to achieve the Sustainable Development Goals.
