Evaluación de test de prensión fina y test de prensión grosera en el Síndrome del túnel carpiano

Abstract

Introducción: La mielopatía cervical (MC) y el Síndrome del túnel carpiano (STC) son patologías neurológicas con sintomatología similar en relación a la afectación de las manos. El objetivo de este estudio es evaluar la efectividad del test de prensión grosera diagnóstico de la MC, aplicado al STC; y comparar sus resultados con otros test diagnósticos del STC. Metodología: 69 pacientes diagnosticados de STC y 43 controles realizaron el test de prensión grosera y tres test diagnósticos del STC. Se midieron las repeticiones hechas por cada paciente y se compararon los valores obtenidos con los controles y la asociación existente entre los test. Resultados: La frecuencia de repeticiones en el test de prensión grosera del grupo experimental es significativamente menor que en el grupo control (16.3 ± 6.6, 28.7 ± 9.2 respectivamente, p-valor <0.001). Existe una asociación significativa del test de prensión grosera con la severidad de la patología (p-valor = 0.008), con el test de prensión fina pulgar-índice (r=0.89, p-valor < 0.001), con el test de prensión fina alternativo (r= 0.73, p-valor < 0.001) y con ambos componentes del Cuestionario de Boston (p-valor = 0.0003 en el caso de la escala de severidad, p-valor < 0.001 en la escala funcional). Conclusiones: El test de prensión grosera sirve como test diagnóstico en la MC y en el STC. Sus resultados están relacionados con los resultados de los test diagnósticos del STC. Por lo tanto, este test no sirve como elemento diferenciador entre la MC y el STC

Background: Cervical myelopathy (CM) and Carpal Tunnel Syndrome (CTS) are both neurological disorders with symptoms in relation to the involvement of the hands. The purpose of this study is to evaluate the effectiveness of the “grip-and-release” test, a diagnostic test in CM, applied to the CTS; and to compare its results with other CTS diagnostic tests. Methods: 69 CTS patients and 43 controls did the “grip and release” test and three CTS diagnostic tests. The results were measured and compared with the controls’ results and the association between the tests. Results: The frequency of repeats in “grip and release” test of the experimental group is significantly lower than in the control group (16.3 ± 6.6, 28.7 ± 9.2, respectively, and p -value < 0.001). There is a significant association of the “grip and release” test with the severity of the disease (p- value = 0.008), with fine grip test thumb-index (r = 0.89, p -value < 0.001), with alternative fine grip test (r = 0.73, p -value < 0.001) and both components of Boston Questionnaire (p- value = 0.0003 in the case of the severity scale, p -value < 0.001 in the functional scale). Conclusions: The “grip and release” test can be used as diagnostic test for CM and CTS. Its results are related with the CTS diagnostic tests’ results. Therefore, this test can’t be used as a differentiator between the CM and the CTS.
