Cuidados centrados en el desarrollo: Intervenciones de enfermería en el recién nacido prematuro

Abstract

La Neonatología es una especialidad que ha ido evolucionando paralelamente al desarrollo tecnológico. Hoy en día, se sigue el modelo de cuidados NIDCAP (Newborn Individualized Developmental Care and Assessment Program) que surgió gracias a la Dra. Als, pionera en los Cuidados Centrados en el Desarrollo (CCD). Ella justificó la importancia del entorno para el correcto neurodesarrollo del recién nacido prematuro. En el siguiente trabajo, se pretenden mostrar las recomendaciones que existen actualmente sobre el cuidado de este tipo de paciente, a través de una revisión bibliográfica de manera sistemática. A su vez, se van a exponer las intervenciones que los profesionales de enfermería deben asumir para favorecer su neurodesarrollo. Si observamos las situaciones que se dan en este momento, encontramos que en muchos de los hospitales de España no se llevan a cabo todas las medidas posibles que, la evidencia científica ha demostrado que ejercen un efecto positivo en la correcta evolución de estos. Concretamente, en el Complejo Hospitalario de Navarra (CHN) existe una heterogeneidad en la práctica clínica debido a la ausencia de un documento que recoja los pasos de los CCD. El objetivo general del trabajo consiste en proponer un compendio de procedimientos según el modelo NIDCAP, que permita unificar las intervenciones en la Unidad de Cuidados Intensivos Neonatales del CHN.

Neonatology is a specialty that has evolved in parallel with technological development. Nowadays, it is used to follow a care model called NIDCAP (New born Individualized Developmental Care and Assessment Program), that emerged thanks to Dra. Als, a pioneer in Centered Development Care (CCD). She justified the importance of the environment for the correct neurodevelopment of the premature newborn. In the following work, we intend to show the recommendatios that currently exist about the care of this type of patient, through a systematic literature review. At the same time, it is explained the interventions that nursing must take to promote their neurodevelopment. If we observe the situations that occurs at this moment, we find that in many of Spanish hospitals are not carried out all the possible measures that the scientific evidence has shown they exert a positive effect in the correct evolution of these. Specially, in Complejo Hospitalario de Navarra (CHN), there is a heterogeneity in clinical practice due to the absence of a document that includes the explanation of the CCD. The general objective of this work consists of proposing a compendium of procedures according to the NIDCAP model, which allows unifying the interventions in the Neonatal Intensive Care Unit of CHN.
