Ventilación mecánica no invasiva en el adulto: guía de enfermería

Abstract

La ventilación mecánica no invasiva (VMNI) es un soporte ventilatorio con presión positiva cuya principal aportación es que no precisa de intubación endotraqueal, por lo que se evitan las complicaciones derivadas de esta; manteniéndose los mecanismos de defensa de la vía aérea superior, el habla y la deglución. Es una técnica que se debe aplicar de forma precoz y debe ser manejada por personal experimentado en las unidades donde el paciente pueda estar adecuadamente monitorizado. Gracias al desarrollo del conocimiento de este soporte ventilatorio, unido a la progresiva adquisición de experiencia por parte de los profesionales sanitarios y a los avances tecnológicos, está siendo adoptada como primera opción ante la necesidad de ventilación mecánica en las unidades de cuidados intensivos (UCI). Las características especiales de esta terapia están relacionadas tanto con la inversión de las presiones ventilatorias, como con la selección de pacientes, interfaces y opciones terapéuticas. No obstante, cuenta con una serie de posibles complicaciones que pueden conducir al fracaso de la técnica si no se establece un estrecho control sobre la terapia, el paciente, el aparataje y los cuidados asistenciales recogidos en la guía práctica propuesta.

The non-invasive mechanical ventilation (NIMV) is a ventilator support with positive pressure whose main contribution is that it does not need endotracheal intubation, so avoid complications arising from this; keeping the defense mechanisms of the upper airway, the speech and swallowing. It is a technique that should be applied at a prematurely stage and should be handled by experienced personnel in units where the patient can be properly monitored. Thanks to the development of knowledge of this ventilator support, together with the gradual acquisition of experience by health professionals and technological advances, it is being adopted as a first option when there is the need of mechanical ventilation in intensive care units (ICU). The special characteristics of this therapy are related to both the investment of ventilation pressures, as with the selection of patients, treatment option and interfaces. However, it has a series of possible complications that can lead to the failure of the technical if not set a tight control on therapy, the patient, the equipment and medical care, contained in the Guide to Practice proposed.
