    Are previous suicide attempts a risk factor for completed suicide?

    Antecedentes: el intento previo de suicidio es un factor clínicamente relevante para el suicidio consumado. En este trabajo se comparan las personas que se suicidaron en su primer intento con quienes lo hicieron tras intentos previos. Método: revisión de las historias clínicas del Servicio Navarro de Salud-Osasunbidea (2010-2013). Resultados: de los 166 casos, el 31,9% (n = 53) presentaba algún intento previo. De estos 53, el 65,3% modificó el método de suicidio. Las mujeres presentaban más intentos (χ2 = 14,3; g.l. = 3; p = ,002). Se identificaron tres submuestras: los diagnósticos de trastornos de personalidad (90,9%; n = 10) y las mujeres menores de 51 años con diagnóstico de trastornos afectivos, de ansiedad o por consumo de sustancias (82,4%; n = 14) presentaban los mayores porcentajes de intentos. Las personas sin diagnóstico psiquiátrico, con trastornos psicóticos u orgánicos fueron quienes menor proporción de intentos presentaban (13,2%; n = 10) junto con las personas mayores de 51 años diagnosticadas de trastornos afectivos, de ansiedad o por consumo de sustancias (22,5%; n = 9). Conclusiones: los intentos previos son factores de riesgo únicamente en submuestras clínicas específicas. Los programas de prevención e intervención deberían confeccionarse teniendo en cuenta este resultado.

    Therapeutic progression in abused women following a drug-addiction treatment program

    This study explored the prevalence of victims of abuse and the therapeutic progression among women who sought treatment for drug addiction. A sample of 180 addicted Spanish women was assessed. Information was collected on the patients’ lifetime history of abuse (psychological, physical and/or sexual), socio-demographic factors, consumption variables and psychological symptoms. Of the total sample, 74.4% (n = 134) of the addicted women had been victims of abuse. Psychological abuse affected 66.1% (n = 119) of the patients, followed by physical abuse (51.7%; n = 93) and sexual abuse (31.7%; n = 57). Compared with patients who had not been abused, the addicted women with histories of victimisation scored significantly higher on several EuropASI and psychological variables. Specifically, physical abuse and sexual abuse were related to higher levels of severity of addiction. Regarding therapeutic progression, the highest rate of dropout was observed among victims of sexual abuse (63.5%; n = 33), followed by victims of physical abuse (48.9%; n = 23). Multivariate analysis showed that medical and family areas of the EuropASI, as well as violence problems and suicide ideation, were the main variables related to physical and/or sexual abuse. Moreover, women without abuse and with fewer family problems presented the higher probability of treatment completion. The implications of these results for further research and clinical practice are discussed.

    Seguimiento telefónico en la prevención de la conducta suicida

    En este capítulo se presenta la comparación de un programa de seguimiento telefónico estructurado añadido al tratamiento habitual sobre la repetición de la conducta suicida en una muestra de pacientes atendidos por intento de suicidio en urgencias de psiquiatría, con un seguimiento de un año. El grupo de comparación es una cohorte de pacientes anterior a la inclusión del programa de seguimiento telefónico.

    Violent behaviours in drug addiction: differential profiles of drug-addicted patients with and without violence problems

    This study explored the prevalence of violent behaviours in patients who are addicted to drugs. A sample of 252 addicted patients (203 male and 49 female) who sought outpatient treatment was assessed. Information on violent behaviours, socio-demographic factors, consumption factors (assessed by the EuropAsi), psychopathological factors (assessed by SCL-90-R) and personality variables (assessed by MCMI-II) was collected. Drug-addicted patients who were associated with violent behaviours were compared on all variables to patients who were not associated with violent behaviours. The rate of drug-addicted patients with violent behaviours in this sample was 39.68% (n=100). There were significant differences between the numbers of patients who did and did not demonstrate violence on some variables. Patients with violence problems were younger than those without violence problems and were more likely to report having been a victim of abuse. Moreover, they were significantly more likely to have experienced an overdose and showed a significantly higher score on several EuropAsi, SCL-90-R and MCMI-II variables. According to these results, patients with violence control problems present with both a more severe addiction and several comorbid problems. The implications of these results for further research and clinical practice are discussed.

    Differential profile of addicted patients depending on violent behaviours and/or criminal acts

    This study explored the prevalence of violent and/or criminal behaviours in drug-addicted patients. A sample of 252 drug-addicted patients who sought treatment was assessed. Information on violent behaviours, criminal acts, socio-demographic factors, consumption factors, psychopathological factors and personality variables was collected. The sample was divided into four groups according to the presence of violence and/or criminal behaviours. There were significant differences between the groups on some variables. In general, patients associated with both violence and criminal behaviours showed a greater severity in drug consumption and maladjustment variables, as well as a higher rate of treatment dropout and re-entry.

    Comorbilidad psicopatológica en el alcoholismo: un estudio descriptivo

    En este estudio se lleva a cabo un análisis del perfil de bebida y de la comorbilidad psicopatológica en 50 pacientes alcohólicos que acuden en busca de tratamiento a un programa ambulatorio de Proyecto Hombre de Navarra. Para ello, se lleva a cabo un estudio ex post facto, de carácter retrospectivo y con un grupo cuasi control. Se utilizan los criterios diagnósticos del DSM-IV-TR para la dependencia alcohólica, el Müncher Alkoholismus Test (MALT), para valorar la gravedad del alcoholismo, y el SCL-90-R, como medida de la sintomatología asociada. Los resultados obtenidos muestran la presencia de numerosa sintomatología psicopatológica, con elevaciones significativas en la mayoría de las dimensiones del SCL-90-R, tanto en los hombres como en las mujeres de la muestra. La comparación con las muestras normativas refleja que los alcohólicos de la muestra presentan más síntomas psicopatológicos que la población normal, pero menos que la población psiquiátrica. Asimismo, la gravedad del alcoholismo se relaciona de forma significativa con la mayor presencia e intensidad de comorbilidad. Por último, se comentan las implicaciones de este estudio para la práctica clínica y para la investigación futura.

    Substance use disorder and lifetime suicidal behaviour: a scoping review

    Background: Suicidal behaviours are prevalent in substance use disorder (SUD) patients. These behaviours have a negative impact on the psychopathological profile of these patients. However, few studies have evaluated suicide as a continuum (i.e. starting with ideation, followed by attempts and ending with death) and its evolution during treatment. The aim of this scoping review was to explore what is known about suicidal behaviour in individuals undergoing clinical treatment for SUD. Methods: The PsycINFO, Web of Science (Core Collection and MEDLINE), Scopus, and Cochrane Library databases were searched. After screening the records based on eligibility and exclusion criteria, 30 studies were ultimately selected for inclusion. Results: Most of the studies were conducted in the USA and Europe. Only three studies evaluated suicidal behaviour with a specific instrument. The prevalence of suicidal ideation (SI) in people being treated for SUD ranged from 20 % to 62.2 %, and the prevalence of suicide attempts (SA) ranged from 15.8 % to 52.1 %. Only one study reported death by suicide. Conclusions: Despite the high prevalence of suicidal behaviours and their harmful consequences, the assessment of this phenomenon is scarce and heterogeneous. There is a need to assess suicidal behaviour with standardized criteria in order to develop tailored SUD treatment.

    Empatía, cognición social y calidad de vida subjetiva en esquizofrenia

    Fundamento. Las personas con diagnóstico de esquizofrenia presentan déficits en empatía que se han relacionado con su pobre funcionamiento psicosocial. Los objetivos fueron: 1) analizar la relación entre empatía, cognición social, y calidad de vida en sujetos con diagnóstico de esquizofrenia y otros trastornos psicóticos 2) evaluar estas variables según la gravedad del cuadro clínico Material y métodos. Participaron 41 pacientes divididos en dos grupos según el nivel de gravedad del cuadro clínico. Se aplicó una batería de pruebas: el Índice de Reactividad Interpersonal (IRI), escala GEOPTE de Cognición Social para la psicosis, Instrumento de Evaluación de la Calidad de Vida de la Organización Mundial de la Salud – Versión breve (WHOQOL-BREF), Escala de Impresión Clínica Global (CGI), Escala para la evaluación de Síntomas Positivos (SAPS) y Síntomas Negativos (SANS). Resultados. Las subescalas angustia personal y fantasía del IRI presentaron una relación directa con la escala GEOPTE, e inversa con varias dimensiones de calidad de vida del WHOQOL-BREF. La escala GEOPTE, tuvo una relación inversa con todas las dimensiones de la calidad de vida evaluadas. El grupo levemente enfermo obtuvo puntuaciones significativamente menores en fantasía (15,44 vs. 20,12; p=,001) y significativamente más elevadas en salud psicológica (58,63 vs. 45,40; p=0,017) y ambiente (67,00 vs. 53,68; p=0,006). Conclusiones. Se halla relación entre empatía, cognición social y calidad de vida subjetiva en personas con diagnóstico de esquizofrenia en programas de rehabilitación psicosocial, Asimismo, destaca la existencia de diferencias relevantes en estas variables según el nivel de gravedad del cuadro clínico

    Group and individual change in the treatment of drug addictions: a follow-up study in therapeutic communities

    The pre-post treatment change of 112 patients in two therapeutics Spanish communities is described. The Addiction Severity Index (ASI) was used at intake and at the end of the treatment program. Results of the treatment program were evaluated by applying composite scores (CS) of the ASI, and the evolution of each patient was evaluated using the Reliable Change Index (RCI). 69.7% of the sample completed treatment, and 30.3% dropped out prematurely. At intake, the percentage of people who could improve in the different areas of the ASI ranged between 35.1% for alcohol consumption and 95.3% for family relationships. At the follow-up, the percentage of subjects who showed significant statistical improvements in the different areas varied between 7.9% in family relations and 66.7% in alcohol consumption. The percentage of patients who deteriorated was less than 10% for all variables. Implications for further research and clinical practice are commented upon.

    Effectiveness of a telephone prevention programme on the recurrence of suicidal behaviour. One-year follow-up

    People who have attempted suicide are considered a risk population for repeating the behaviour. Therapeutic interventions, such as telephone follow-up programmes (TFPs), are promising but more evidence for its efficacy is needed. In this multicentre, open, ex-post-facto, pre/post, one year prospective study, a previous cohort discharged from the emergency department for a suicide attempt (SA) and given routine treatment (n=207) was compared with a similar group who received the same intervention plus a structured TFP of six calls (n=203). At one year of follow-up, the efficacy of the TFP at preventing SA was assessed. A total of 53.2% (n=108) of the patients finished the TFP. A total of 20.3% (n=42) of the routine treatment group and 23.6% (n=48) of the TFP group reattempted at least once in the follow-up period (χ2=0.7;df=1;p=.412). However, in both groups, different subsamples of patients who presented extreme risk of SA at follow-up (0-57%) were identified. In the TFP group, the recurrence of suicidal behaviour was lower in patients admitted after the index attempt and in those who had more severe psychopathological symptoms, but not in the other profiles. Thus, this study has identified a specific profile of patients who could benefit from a brief-contact intervention.
