Usos y cuidados de los catéteres de línea media (CLM)

Introducción: los catéteres de línea media (CLM) son dispositivos de 8-25 cm que se introducen a través de una vena profunda (generalmente la basílica) hasta la axila. Se introducen por profesionales de enfermería gracias a la técnica Seldinger con guía ecográfica. Dicha técnica consigue reducir el número de venopunciones y el sufrimiento del paciente, aumentando la satisfacción del paciente, allegados y profesional sanitario, por lo que en estos últimos años se ha visto un aumento en el número de analizaciones de dichos dispositivos. Fueron muy populares en la década de los 80, pero debido a varias reacciones de hipersensibilidad su uso se vio reducido, hasta que se logró una mejoría en su composición y en el éxito de la canalización en el primer intento. Objetivo: el principal objetivo de esta revisión bibliográfica es identificar las indicaciones de uso y los cuidados a realizar en los catéteres de línea media. Material y métodos: se ha realizado una revisión bibliográfica sistemática con metodología narrativa, realizando búsquedas en bases de datos como Pubmed, Cinahl, Cuiden o Google Scholar. Tras aplicar los criterios de inclusión y exclusión se han empleado nueve artículos para realizar la revisión. Discusión y conclusiones: según los estudios analizados, las dos principales indicaciones de uso de dichos dispositivos son el acceso venoso difícil y la necesidad de poseer de un acceso venoso para tratamiento intravenoso con antibióticos o cualquier otro medicamento. Los profesionales de enfermería deben de realizar los cuidados del dispositivo adecuadamente para mantener el dispositivo en condiciones adecuadas. Los autores indican que hace falta investigar más en este campo, además de resaltar la importancia de realizar investigaciones dentro de nuestro sistema sanitario.

Introduction: midline catheters (MVC) are 8-25 cm devices that are introduced through a deep vein (usually the basilic) to the axilla. They are introduced by nursing professionals using the Seldinger technique with ultrasound guidance. This technique reduces the number of venipunctures and the patient's suffering, increasing the satisfaction of the patient, the family teams and healthcare professional, which is why in recent years there has been an increase in the number of cannulations of these devices. They were very popular in the 80's, but due to several hypersensitivity reactions their use was reduced, until an improvement was achieved in their composition and in the success of cannulation in the first attempt. Objective: the main objective of this bibliographic review is to identify the indications for use and the care to be performed on midline catheters. Material and methods: a systematic literature review was carried out using narrative methodology, searching databases such as Pubmed, Cinahl, Cuiden or Google Scholar. After applying the inclusion and exclusion criteria, nine articles were used for the review. Discussion and conclusions: according to the studies analyzed, the two main indications for the use of these devices are difficult venous access and the need to have venous access for intravenous treatment with antibiotics or any other medication. Nursing professionals must perform the care of the device properly to maintain the device in adequate condition. The authors indicate that more research is needed in this field, as well as highlighting the importance of conducting research within our healthcare system.
